Do the HMOs participate in the payment for the visit in the private clinic

Yes, as long as you have Supplementary insurance.
1. “MACABI-magen zahav” - the visit costs 900 NIS and the HMO’s refund is 622 NIS (up to 3 visits a year) (down payment of 278 NIS).
2. “MEUHEDET adif/si” - The HMO’s refund is 80% of the visit’s cost.
3. “KLALIT mushlam/Platinum - The down payment is 120 NIS.
4. “LEUMIT ve’od/zahav” -the visit costs 900 NIS and the HMO’s refund is 80% of the visit’s cost.
5. Private insurers (such as HAREL, HA FENIX, MIGDAL, BITUH YASHIR, MENORA, KLAL, DIKLAH, etc.) recieve 80% refund of the visit’s cost according to the different policies.

If I want Dr Ben Chaim to operate my child- what should I do

At public hospital you can’t choose your surgeon.
If you would like to choose Dr Ben Chaim as your surgeon, you should come for an examination at one of the doctor’s clinics. After the examination, if needed- a date for the surgery will be scheduled at the private hospital.

Where is the surgery performed

The surgeries are performed at Assuta hospital at Ramat Hahaial – Habarzel street 20, Tel Aviv. (The Tel Aviv Assuta hospital is the largest and most modern private hospital in Israel).

What are the certificates(17 form) required to cover the payment for the different surgeries and how can I get it?

1.If your insurer is private (such as: Harel, Menora, Hafenix, Migdal, Bituh Yashir, etc..)
You should get two cetificates from the insurer:
1. for Assuta hospital.
2. for Dr Ben Chaim - who is in the surgeons’ list of the above insurers.
(in most cases, according to the different policies-the private surgery doesn’t cost at all).
2.If you are a member of Macabi, you should get a cetificate (form 17) from the HMO for the surgery. In addition, you will have to pay the down payment required for the hospitalization and also for the surgeon. The down payment will be paid at the reception for the surgery at Assuta hospital.
3.If you are a member of Meuhedet, Leumit, Klal Bituh Supplementary insurances, you should get a cetificate (form 17) for the hospitalization at Assuta hospital and pay separately the surgeon’s fee to Dr Ben Chaim(in cash or check). (with the tax invoice, you can get the refund from the Supplementary insurance).
4.If you are a member of Klalit Mushlam Supplementary insurance, you should pay the down payments for the surgery according to Klalit Mushlam’s rates. The payment will be paid at Assuta’s hospital reception in the surgery’s day with any method of payment.

*in order to get the cetificates, you should contact the insurer with the recommendation for surgery letter(consultation’s summary) from Dr Ben Chaim.

Does Dr Ben chaim perform ritual circumcisions for babies

No. Dr Ben Chaim doesn’t perform ritual circumcisions for newborns. However; the Dr does circumcisions and repair of ritual circumcisions for babies, children and even adults. The circumcisions are performed under general anesthetisia at Assuta hospital and Dr ben-Chaim is an expert of that procedure.
